→ The "segmentation gap" -- 74% of sites had a Purdue model on paper but did not enforce it in the firewall.
→ Switch firmware median age: 4.2 years; FortiGate firmware median age: 2.1 years.
→ Remote access pathways averaged 3.4 per site, only 1.6 documented.
→ SNMPv3 adoption: 41% of switches by Q1 2026 (up from 12% in 2023).
→ NetBox source-of-truth adoption: 28% of sites, with 92% adoption in sites that ran a modernization program.
